Interim Healthcare is looking for a Palliative Care Social Worker to join their team located in the St. Louis Park, MN area. -As a Palliative Care Social Worker, you will assist with patients and their families in dealing with life threateningdiseases at all stages (from newly diagnosed thru end of life and in-between). Must be comfortable in providing visits in patient homes, ALFs, LTCF and residential group homes.*To be considered, all candidates are required to hold a graduate level degree in Social Work and be currently licensed as either a LICSW or LGSW in the State of Minnesota.Schedule is a .9 (36 hours per week) Monday - Friday. Starting rate is up to $66 per hour (depending on experience!)Coverage area is within a 40-mile radius of St. Louis Park, MN.
