OverviewM Health Fairview has an immediate opening for a Clinic
Health Guide/EMT to support the M Health Fairview Lakeville Clinic
located in Lakeville, MN.This is full time (1.0 FTE, 80 hours per
two week pay period), benefits-eligible opening.Hiring ProcessIf

will involve telephones, rooming patients, and patient
management/engagement.As a member of the care team, the Clinic
Health Guide advocates and supports quality patient and
family-centered care principles through performing a variety of
high level functions within a care team to support the needs of the
healthcare team and patients/families. The Clinic Health Guide will
gather information and documents to support the patient visit. The
Clinic Health Guide is responsible for adhering to all policies,
procedures and practice guidelines, promoting teamwork with all
members of the health care team, maintaining complete, timely and
accurate clinical documentation for each patient and continually
looking for opportunities to improve processes and workflow. The
role involves listening, understanding, facilitating, applauding,
supporting, motivating and providing feedback to the
patients.Delivers care as delegated by the physician/authorized
practitioner or RN, to assigned patient population, or individual
patient.Follows the patient's plan of care and provides through
basic patient instruction.Prioritizes work based on patient acuity,
available resources, patient/family preferences, provider schedule,
in collaboration with the physician/authorized practitioner and
other health care team members.Performs high and low risk technical
procedures, including medication administration, defined by the
clinical setting.Recognizes when different approaches may be needed
based on age or cultural differences.Motivate patients to achieve
goals that enhance the quality of their lives and improve their
health. Helps patients weigh options, make choices and plan and
identify challenges to help them change for the better. Instructs
patient on health maintenance, disease prevention, and compliance
with patient's treatment plan within their scope of practice as
directed by supervising physician Effectively communicates
compliance and concerns to delegating physician/authorized
practitioner/RN. Coordinates all activities for patient including,
determine additional resources as needed, collaborates with care
team members to ensure continuity of care.Responds appropriately to
emergent situationsAdministers injections or assists with
treatments/procedures under direction of authorized
practitionerIdentifies community resource for health maintenance
and disease prevention to meet individual patient needs
Communicates effectively with other members of the health care
team.Documents care in the medical record. Communicates verbal and
written information in a clear, concise, accurate, and timely
manner. Uses communication approaches appropriate to patient/family
needsRespect and ensures the confidentiality of all information
regarding patients, staff members or Fairview business issues.

QualificationsRequiredEducation:High School graduate or
equivalentExperience:2 years of ambulance or Emergency Department
experience requiredLicense/Certification/Registration:BASIC Life
Support - Red Cross or American Heart AssociationEMT Certification
from an approved institution or training
programPreferredEducation:N/AExperience:3 years of ambulance or
Emergency Department experience
preferredLicense/Certification/Registration:N/A
