Job Description:
Pinnacle Services is seeking a passionate and dedicated CFSS
(Community First Services and Supports) Consultation Coordinator to
join our team. In this role, you will play a critical part in
managing and facilitating comprehensive support services tailored
for individuals with disabilities or special needs. Your expertise
will ensure that our clients receive personalized and effective
consultations, aligning with their individual health goals and
promoting their overall well-being. Coordinate and manage the
delivery of CFSS consultations to clients with disabilities or
special needs. Develop, review, and update individual care plans in
collaboration with clients, families, and healthcare providers.
Ensure compliance with state and federal regulations regarding CFSS
and related healthcare services. Provide training and support to
staff and caregivers on the proper implementation of CFSS
guidelines and practices. Document all client interactions,
consultations, and outcomes accurately and timely in the client
management system. Act as a liaison between clients, families, and
healthcare professionals to ensure a cohesive approach to health
management. Conduct regular follow-ups with clients to assess the
effectiveness of their care plans and make necessary adjustments.
Strong organizational and leadership skills Excellent communication
and interpersonal skills Proficiency in documentation and case
management software In-depth knowledge of CFSS regulations and
healthcare standards Ability to manage multiple cases efficiently
under time constraints Medical Insurance Dental Insurance Vision
Insurance Life Insurance 401k Certified Case Manager (CCM)
preferred First Aid and CPR certification Any relevant
state-specific certifications related to disability services
