II - ACT FSLA: Full-time, Exempt Location: St. Paul, MN Purpose :
The Case Manager II is responsible for providing comprehensive,
community-based services to individuals with severe and persistent
mental illness (SPMI) and/or chemical dependency to live
self-directed and satisfying lives by providing services directly,
minimally using outside resources and diverting hospitalizations by
finding alternative interventions whenever possible. This role
collaborates within an interdisciplinary team to provide direct
client support, manage medications, assist with benefits, and
coordinate care, all aimed at improving client functioning and
overall quality of life. Key Responsibilities : Duties and
Responsibilities : Build trusting relationships with clients to
encourage their active participation in treatment and recovery.
Assist clients in managing their mental health symptoms, daily
living skills, and overall well-being. Ensure timely medication
delivery to clients and monitor adherence to prescribed treatment
regimens. Assist clients in renewing or applying for benefits,
including Medicaid, Social Security, housing assistance, and other
community services. Collaborate with other service providers and
natural supports, ensuring clear communication and coordination of
services. Actively participate in team meetings and care
conferences to discuss client progress and share insights with team
members. Complete required documentation in compliance with
organizational and regulatory standards. Perform other duties as
assigned to support client care and department operations. Promote
an inclusive, diverse, equitable, and respectful work environment.
Required Skills/Abilities : Understanding of mental health
diagnoses, treatment modalities, and the specific needs of
individuals with SPMI. Understanding of community resources, county
benefits, and mental health services. Ability to manage a caseload,
prioritize tasks, and meet deadlines in a dynamic, community-based
environment. Ability to assess and intervene in crisis situations,
utilizing de-escalation techniques and connecting clients to higher
levels of care when needed. Ability to work effectively in a
team-based environment and collaborate with multiple stakeholders
to meet client needs Ability to develop and maintain receptive,
open, and approachable lines of communication with clients and
staff, and provide a friendly environment for visitors. Strong
written and verbal communication skills, including composing,
proofreading, and editing. Presentation and meeting facilitation
skills. Ability to work as a proactive and positive member of a
team to deliver quality services. Ability to maintain
confidentiality of organization, personnel information, and client
data. Ability to use sound judgment and maintain healthy
professional boundaries. Analytical and problem-solving skills.
Ability to meet deadlines and set priorities. Proficiency with a
variety of web-based software and databases, including Microsoft
Office Suite. Working Conditions : This position is community
based, requiring frequent travel to clients’ homes and to various
community locations. Travel may be required to other program sites.
Requires transporting clients in a personal vehicle regularly. This
position has the potential to come into contact with bodily fluids,
communicable diseases, tobacco smoke, and household pets. Prolonged
periods at a desk, working on a computer. Manual dexterity for
tasks such as taking notes, using technological devices for
documentation, or assisting clients with certain activities if
needed. Must be ambulatory and able to negotiate stairways. The
ability to lift and carry heavy weights that could be up to 50
pounds. Education and Experience: Required Has a master’s or other
graduate degree in behavioral sciences or related fields OR Has a
bachelor’s degree in one of the behavioral sciences or related
fields AND has 2,000 hours of experience in the delivery of
services to persons with mental illness OR Has 4,000 hours of
experience in the delivery of services to persons with mental
illness. Preferred 1 year of experience in case management or
providing community-based mental health services. Additional
Requirements: Must not be actively receiving services from Guild
and must not have received services within the last two years. A
valid Class ‘D’ Driver’s License, reliable vehicle, satisfactory
driving record and necessary insurance. The ability to provide
on-call crisis support 24/7 in weekly intervals on rotation.

$55,000-$65,000 annually. The range reflects the minimum and
maximum salary for the position. Compensation will depend on the
candidate’s professional experience, skills, and relevant
